REFERRAL PROCESS
Children are referred for Essential Early Education (EEE) services by parents, physicians, community early childhood programs, and community agencies when there is a concern or question about a child’s development. Referrals are assigned to a case manager who contacts the parent(s)/guardian(s) to discuss their interest in moving forward in the referral process. All of our services are voluntary and require approval from the parent(s)/guardian(s). The referral process may lead to a screening and/or evaluation. If you have any questions or concerns about your child’s development please call our Early Ed office at 802-864-8463 and/or complete the EEE Referral Form. If you would prefer to complete a paper form, that can be located HERE. Once a paper form is completed by the parent, teacher, doctor’s office, or agency, the form can be submitted to: Burlington Early Education Program – 150 Colchester Ave, Burlington, VT 05401 or scurtis@bsdvt.org
Our fax number is 802-864-8452 please note: Someone MUST be in the office and near the fax for the fax to go through. If no one is there to accept the fax, the fax will not go through.
