This form is required for all PreK students.  The form must be completed by the child’s pediatrician and returned to the Burlington Early Education office.

The completed form can be:
Faxed by the parent or doctors office to 802-864-8452,
Scanned and emailed to: cneary@bsdvt.org,
Mailed to: 150 Colchester Ave, Burlington, VT 05401,
OR returned to school in your child’s backpack.
